Parental Consent for School Health-Related Services
Per Senate Bill 12, Texas school districts must have written parent/guardian consent before providing any non-emergency health-related services at school or during school-sponsored activities. Only lifesaving care may be given without consent.
Health-related services may include:
- First aid and illness/injury care
- Vital sign checks (e.g., temperature, blood pressure)
- Vision/hearing screenings
- Diabetes and concussion response
- Medication administration
- Health education, lice/spinal screenings, and more
Services may be provided by nurses or trained school staff (e.g., teachers, coaches, counselors). If you decline consent on this form, your child will not receive care for non-life-threatening issues and must be picked up immediately if ill or injured.
Mobile Vaccine Clinic Thursday, February 13th
Christus Children’s will have a mobile vaccine clinic at Menchaca on Thursday, February 13, 2025 from 9am- noon.
There will be flu vaccines and routine vaccines for ALL ages available.
* If your child turns 4 at the end of January or beginning of February or is on a catch-up schedule, this is your chance to get them their required vaccines without having to miss work or school!
* If you have a child you will be registering for school this is a good opportunity to get them the vaccines they will need to start. I would be happy to review vaccine records before registration events.
* If you have a baby who needs vaccines, you are welcome to attend.
* If you have a child going into 7th grade, this is for you, too.
* Do you or your child need a flu vaccine? (head’s up: this season has been rough and is predicted to go into May!)

FREE Mass Flu Vaccine Clinic
Visit the mass vaccine clinic to get this year’s flu shot and help prevent serious illness!
Please note that this mass vaccine clinic is limited to this year’s flu vaccine. The 2024-2025 COVID-19 vaccine is now available and covered by most insurance. Also, at the end of this month, each household will be eligible to receive 4 free at-home COVID-19 rapid tests by mail through a U.S. Department of Health and Human Services. Families can sign up to receive the 4 free COVID-19 rapid tests by visiting www.COVIDtests.gov. In the meantime, more information can be found at https://aspr.hhs.gov/covid-19/test/Pages/default.aspx.
