Benefit elections will become effective 9/1/2018 (elections requiring evidence of insurability, such as life Insurance, may have a later effective date, if approved). After annual enrollment closes, benefit changes can only be made if you experience a qualifying event (and changes must be made within 30 days of event).
SUMMER ENROLLMENT BENEFITS
FSA: If you currently participate in a Healthcare or Dependent Care FSA, you MUST re-elect a new contribution amount every year to continue to participate.
TRS Medical: All of the TRS medical plans will experience a rate increase effective 09/01/2018. ActiveCare 2 will no longer be available for new enrollees, however, current participants may elect to remain on the plan. The deductibles for ActiveCare 1 HD have increased as well as the out of pocket maximum.
HSA: Southside ISD will no longer be matching the HSA account effective 9/1/2018.
Medical Supplement by Avesis/ SIS
This supplemental coverage helps pay out-of-pocket costs you experience due to deductible and coinsurance of your employer’s medical plan. The available plan options are based on enrollment in your employer’s medical plan.
Health Savings Account (H.S.A.) by HSA Bank
Your school district offers employees who are enrolled in a high deductible health care plan the opportunity to contribute to an H.S.A. to pay for eligible medical, dental and vision expenses. Employee only maximum contribution is $3,450 and Family maximum contribution is $6,850 per year. If you participate in a H.S.A. your FSA will be limited to only dental and vision.
Telehealth by MDLive
This plan allows employees and their household members to call in to a licensed network doctor for non-emergencies such as answers to medical questions and diagnosis for common conditions. Provides 24/7 access to a doctor via the telephone for treatment of common conditions.
Dental Insurance by Cigna
Low PPO Plan- This plan has a $50 deductible for individuals and a $150 deductible for families. Class I expenses are covered at 100%, Class II services are covered at 80%, and Class III services are covered at 25%. The calendar year maximum benefit per person is $1,000. High PPO Plan – This plan has a $50 deductible for individuals and a $150 deductible for families. Class I expenses are covered at 100%, Class II services are covered at 80%, and Class III and Orthodontia (to age 19) are covered at 50%. Orthodontia has a lifetime maximum benefit of $1,000. The calendar year maximum benefit per person is $1,250.
Vision Insurance by Avesis
Vision insurance provides coverage for routine eye examinations and may cover all or part of the costs associated with contact lenses, eyeglasses and vision correction depending on the plan.
Individual Life by Texas Life
This plan provides a level benefit to age 121. Individual policies can be purchased on the employee, their spouse, children and grandchildren.
Long Term Disability by The Hartford
Designed to provide a monthly income to an individual that is disabled due to an accident or illness. There are multiple plans available from the 1st day of disability or as late as the 90th day. All new or increases in coverage are subject to pre- existing conditions exclusions.
Accident Insurance by Voya
Accident Insurance is designed to be a supplement that pays benefits directly to you. Coverage is available in High or Low plans.
Critical Illness Insurance by Reliance Standard
The Group Critical Illness product provides a lump- sum benefit upon the diagnosis of not only one covered illness, but for each covered illness.
Cancer Insurance by Humana
Cancer insurance offers you and your family supplemental insurance protection in the event you or a covered family member is diagnosed with cancer. Benefits are paid to you to help with medical and non-medical expenses associated with cancer treatment.
Voluntary Term Life with AD&D Insurance by OneAmerica
Voluntary term life insurance is available for employees up to the lesser of 5 times salary or $500,000.00. All increases in coverage or new elections are subject to evidence of insurability. Eligible expenses must be incurred within the plan year (09/01/18 – 08/31/19) and contributions are “use it or lose it.” Direct deposit is available. The annual maximum is $2,650 per plan year.
