Join us on April 29th from 2 pm to 5:30 pm at Heritage Elementary for the last meeting of the school year.
Whether you have questions about your current Medicaid/Medicare application or need help with a new application, we will be at Heritage to assist your family.
This is a come-and-go event where representatives from WellPoint and Generations Insurance Group will help you with your application or answer any questions and concerns you may have. Please note that the application can take anywhere from an hour to 2 hours.
Be sure to bring the following documents to complete your application:
- Proof of ID
- Proof of income (last 30 days)
- Proof of address
- Social Security Number
- Birth Certificate
- Proof of Pregnancy (If applicable)
WellPoint offers Medicaid application assistance for low-income children and women.
Generations Insurance Group offers Medicare, Marketplace, and other insurance options for those who don’t qualify for Medicaid, including seniors, individuals with disabilities, and those transitioning out of their current Medicaid coverage.
As a reminder, these services are free for our community.
