Families and staff, join us on February 20th from 2pm to 5:30pm at Matthey Middle School for help with your Medicaid application. Whether you have questions about your current application or need help with a new application, we will be at Matthey Middle School to assist your family.
This is a come-and-go event where representatives from WellPoint will work on your application, asking you questions as needed. Please note that the application can take anywhere from 30 minutes to an hour.
Be sure to bring the following documents to complete your application:
- Proof of ID
- Proof of income (last 30 days)
- Proof of address
- Social Security Number
- Birth Certificate
- Proof of Pregnancy (If applicable)
Medicaid provides health coverage for low-income children, families, seniors, and people with disabilities.
