Southside ISD Offers Low-Cost Physical Exams May 3rd

Attention Future Cardinal Champion Athletes:

On Wednesday May 3 from 5:00 pm-6:30pm,  incoming Middle School 7th graders and ALL 9th through 12th grade male and female student athletes are encouraged to take advantage of this great opportunity. 2017-2018 Sports Physical Exams will be taking place at the Southside High School Gym for just $10.00 cash only. Athletes will be seen on a first come, first serve basis. Physicals are offered to students wanting to participate in athletics.

In order to compete the Southside Athletic Department requires all athletes to undergo a Sports Physicals for the 2017-2018 School Year.

WHO NEEDS A PHYSICAL: High School and Middle School Male & Female Athletes

( Incoming Middle School 7th graders and ALL 9th through 12th grade)

WHEN: Wednesday May 3, 2017 

TIME: 5:00 – 6:30 p.m.

WHERE: Southside High School Gym

FEE:  $10.00 Cash only (No debit cards, credit cards, or checks accepted.)

Students must bring a completed Medical History form signed by parent/guardian on the day of scheduled physical date to have a sport physical exam performed.

Students will receive a receipt the day of the exam, and are encouraged to keep it for their personal records. Students will not go home with the participation packet after the exam.

Parents may request a hard copy of the physical exam through the Athletic Training Office after May 12th 2017.  For more information contact the Southside Athletic Training office (210)882-1606 ext. 1532.

Per Southside Athletic Policy – High School athletes must have an updated physical every year to participate in any practice, before, during, or after school (both in-season and out of season games/matches.)

Per Southside Athletic Policy – Middle School athletes must have an updated physical on file, no older than 1 year, to participate in athletics at the middle school participation level, before they can participate in any practice, before, during or after school (both in-season and out-of season games/matches.)

 If an athlete was injured during participation, they must have an updated physical.

Students may pick up an Athletic Participation Packet that includes the physical form, from the Athletic Office, High School, and Middle School Athletic Training rooms; obtain a packet from their Coach, or download a copy from the Southside ISD Website under Departments -> Athletics.

Athletic Participation Packets must be turned into Southside Athletic Trainers only. Do not turn completed athletic participation paperwork into a coach, athletic office or campus administration front office.

Participation packets can be turned in at the High School or Middle School Athletic Training Facility during posted office hours.
